1. Pursuant to the provisions of reference (a) Subject,
hereinafter referred to as Petitioner, filed enclosure (1) with
this Board requesting, in effect, that the applicable naval record
pe corrected to establish entitlement to an Enlistment Bonus for
Source Rate (EBSR).
2. The Board, consisting of Ms. Bianchi, Mr. Vogt, and Mr. Ruskin,
reviewed Petitioner's allegations of error and injustice on 7
October 2014 and, pursuant to its regulations, determined that the
corrective action indicated below should be taken on the available
evidence of record. Documentary material considered by the Board
onsisted of the enclosures, naval records, and applicable

3. The Board, having reviewed all the facts of record pertaining
to Petitioner's allegations of error and injustice, finds as
follows:
a. Before applying to this Board, Petitioner exhausted all
administrative remedies available under existing law and
regulations within the Department of the Navy.
b. In correspondence attached as enclosure (2), the office
having cognizance over the subject matter addressed in Petitioner’s
application has commented to the effect that the request has merit
and warrants favorable action.
CONCLUSION
Upon review and consideration of all the evidence of record, and
especially in light of the contents of enclosure (2), the Board
finds the existence of an injustice warranting the following
corrective action.

That Petitioner's naval record be corrected, where appropriate, to

b. and Block 19.f. (Date Signed) of the DD Form 4/1 (Oct 2007), are
modified to read "20110830 Ti: 10 S56" vice "90120320 11:10:56."
b. Annex A to the DD Form 4 signed “12 April 2012” and dated
“30 August 2011” is now designated Annex B. This change
establishes continuity in Petitioner’s record, and establishes
entitlement to the following enlistment guarantees:
Option (1) Advanced Technical Field Special Warfare Operator
(ATF/SO 6YO) Challenge Program
Option (2) Enlistment Bonus for Source Rate (EBSR) $15,000 bonus
(PERS-811), PSD Charleston, or
c. The Navy Personnel Command
EB entitlement as appropriate
